Vital. (Please consider, that the documentation may not necessarily be paper based, it can be electronic, or embedded in the application code in the form of comments). An essential requirement is to document the QA practices so that they can be repeated whenever required. All the documents such as Specifications, requirements, physical and logical designs, business rules, business logic, inspection reports, review results, configurations, code modifications, test and retest plans, test cases, bug reports, bug fix reports as well as user manuals, etc. should be documented for future reference and record keeping purposes.
Ideally, there should be a system defined to easily search for information and understanding which type of document will have the required information. The documentation should incorporate change management processes.
